Hi I think it would be better if you will just request for change sim w/ same no. at Globe store.If this is due to defective sim. just bring 2 Valid ID's and the actual sim that has problem.
If this is due to LOst. You need to submit a Notarized Affidavit of Loss and 2 valid id's.
