I'm sure Globe would have the appropriate details of your account (account name, account number, phone number, subscription) and just go to your nearest Globe business office and mention the details you know and once they trace the lacking details they would gladly replace your sim if they have stock.
You would have to bring a valid ID though to very your identity and compare it to their records for verification
I experienced losing my sim. So, globe replaced it. But I provided a valid ID and they asked several information.
