Try to check the PUK code at the back of the simcard tray. you might see PUK1 and PUK2. then try it.

if SIM Bed is no longer available, you should visit any nearest Globe Store and request for SIM replacement with the same mobile number...
don't forget to bring the following
- 1 valid ID
- actual SIM card
This should be free of charge... should you wish to have your phonebook entries to be transferred to your new SIM, ask them if they can do so since not all Globe Store has the SIM Copier.
For PUK locked sim cards, there are multiple ways for you to retrieve and/or resolve it:
Look for your sim-bed. This is the vinyl / plastic structure that holds a sim card before you take it off and insert on your phone. If you do not have this, you can still retrieve via the next option.
Call the hotline or visit any globe business center with customer service. Our customer service specialist should be able to help you with retrieving this information. If you have lost your sim bed, make sure that you bring along with you the active sim card so they could verify that the account is yours. Also, bring 1 valid ID at the store for validation purposes.
If you really could not retrieve through the following, you may now go ahead and have the sim replaced at the customer service counter at any globe store, again, just bring one valid id for verification purposes.

Since the above mentioned concern was posted under Globe Prepaid Mobile, there's no need to call the hotline since they don't have any means to retrieve the PUK codes for prepaid account. To visit any nearest Globe Telecom Business Center is the resolution instead if and only if the sim bed/tray was lost.
