you call globe hotline 211, for postpaid subscriber, few verification then the customer service will tell you to write down your PUK Code. be ready with your pen and paper
For Postpaid, you may retrieve your PUK code via MyAccount. Just register or login at http://www.globe.com.ph/myaccount to do so.
For Prepaid, kindly proceed to the nearest Globe Store and request for a SIM replacement with the same number. Just bring the actual blocked SIM card and 2 original valid IDs.
